At the Alberta Northwest Palliative Care Society, everything we do comes back to life, compassion, and care. We believe palliative care is not just about end of life, but about living well with support, dignity, and connection at every stage and any age of a progressive life limiting-illness.
Our Circle of Care program brings this belief to life.
Circle of Care is a community-based, in-home palliative support program designed to help individuals living with a progressive life-limiting illness at any age and any stage remain in the comfort of their own homes, surrounded by what matters most. Supported by trained volunteers, the program complements existing healthcare services by focusing on the human side of care and responding to real, day-to-day needs beyond clinical care.
Many individuals wish to receive care at home, but the reality can feel overwhelming for both them and their caregivers. Circle of Care helps bridge that gap by offering compassionate presence, practical support, and meaningful connection.

The program is guided by three core pillars:
Companionship is at the heart of what we do. Volunteers offer friendly visits, meaningful conversation, and a listening ear, helping reduce isolation and providing emotional and caregiver support through simple, consistent presence.
Navigation helps individuals and families better understand what supports exist and where to turn. Volunteers connect people to community resources and services, helping ease confusion and overwhelm without stepping into clinical roles.
Advance Care Planning supports conversations around values, wishes, and future care decisions. Volunteers help create space for these important discussions, encouraging individuals to think about what matters most and communicate those wishes with their loved ones and care teams.
A progressive life limiting-illness can be deeply isolating. At its core, Circle of Care exists to ensure that no one has to face it alone. By meeting people where they are at and building compassionate community support, the program helps individuals live as fully as possible, for as long as possible.
The program reflects the unique needs of Northwest Alberta and the voices of those with lived experience.
As with all of our programs, Circle of Care is grounded in the belief that community matters, and that together, we can create a circle of care around every person and family who needs it.
